When regulators prohibit individuals from industry participation, the failure trace runs backward through every institution that granted them access without documented fitness review. The missing artifact is the participation-authority decision itself: who approved this person's role, on what evidence, subject to what ongoing review schedule, and escalated to whom when red flags emerged. Institutions that treat hiring and role assignment as informal acts rather than governance decisions lose the ability to demonstrate due diligence when enforcement arrives. Every decision to grant participation authority is a decision that deserves engineering rigor — owner, rationale, evidence, outcome — produced at the moment of approval and revisited on a documented cycle.
